Fees for clinical services are provided by the office and may vary based on your clinical needs. Estimates are not guarantees of final cost. You are responsible for amounts not covered by insurance.

Insurance & pre-determinations

If you would like us to confirm participation with your dental insurance, email your name, date of birth, and photos of your insurance card (front and back) to hello@blancaesthetics.com.

For out-of-network coverage, allow up to two weeks for a pre-determination when available/appropriate.
